Information from your medical records can be used by the NHS to improve medical services.
Your care information along with your NHS number and postcode, but not your name, is shared by a secure system and used by healthcare staff to provide information to improve your NHS care and treatment. This secure system is called care.data and will extract this information from the practice database.
The proposed national roll-out of the care.data programme has been extended until Autumn 2014 to raise awareness, listen and act on the views of the public, patients and key stakeholders and to allow further discussion on the risks and benefits involved.
If you are happy for your care information to be used to improve NHS service, you do not have to do anything.
However, if you have concerns or do not wish for your data to be included, please contact the surgery to discuss this further.
